Medical Coverage During Travel
INF Premier is a limited pre-fixed visitors insurance plan that provides medical insurance coverage and emergency services for non US residents traveling to the USA, Canada and Mexico. The plan also includes pre-existing conditions coverage up to the stated eligible amount in the policy. After the deductible is satisfied, the plan will pay up to the stated eligible amount for covered charges.
The following benefit table shows the pre-fixed amounts the insurance carrier will pay for covered services. If the bill exceeds the pre-fixed amounts covered, the difference will be the insured’s responsibility.

What deductible should I chose?
The deductible is per incident for INF Premier. Please keep in mind that the deductible has to be satisfied completely before the insurance carrier will pay the pre-fixed amount for covered eligible expenses. A lower deductible is recommended.

Renewability
Renewals are not guaranteed. This policy is a short term duration policy renewable at the option of the insurer. If you are able to renew your policy, there will be a $5 administration fee included in your premium.
Cancellation
1) Refunds are only available for cancellation requests submitted prior to effective start date
2) $25 cancellation fee will apply
3) $10 membership and $5 administration fee is non-refundable
4) Premium is fully earned after the policy effective date
5) Written request or email is required for any cancellations up to 24 hours before the effective date

Q1. When is the pre-existing deductible applied?
If you have had any medical issue within the past 12 months, treatment of such issue will be subject to the pre-existing condition maximum. Coverage for Pre-Existing Conditions are subject to the pre-existing deductible and policy maximum. The deductible will be applied each time you utilize the benefit.
Coverage for pre-existing conditions are up to the stated amount on your policy certificate.
Q2. When is the new sickness deductible applied?
If you have Accident or New Sickness the coverage is subject to the new sickness deductible. INF Premier is a limited plan and the deductible is per injury/illness . The deductible will need to be fully satisfied before the insurance company will pay for any covered medical treatment and/or services.
Q3. Am I eligible for INF Premier?
Any Non-US citizen, U.S Expatriate and their dependents. INF Premier will provide coverage for travel to the United States, Canada and Mexico. This plan is eligible for age groups 0 to 99.
Q4. What information do I need to purchase the plan?
You will need names, date of birth/s, passport number/s, home country address and a valid credit card.
Q5. When will I receive my ID card?
After enrolling, you will receive an email confirmation containing an attachment for ID cards, certificate of insurance and visa letter.
Q6. Can I extend the plan if I stay longer?
You are able to extend the INF Premier plan for up to 364 days total prior to the policy’s expiration date. INF will send email, text, and phone call reminders to you 1 week & 1 day before the policy expiration. There is a $5 extension administration fee.
Q7. Can I cancel the plan?
You are able to cancel this plan for a full refund (minus a $25 cancellation fee) before the start date of the coverage. Cancellation forms must be completed in writing and faxed to INF at 408-520-4967. Application fees ($15) are non-refundable. After the policy start date, the premium is fully earned and therefore nonrefundable.
Q8. Will this plan cover pre-existing conditions?
INF Premier is a limited pre-fixed plan that offers coverage for pre-existing conditions. INF Premier will cover for pre-existing conditions up to the set amount in the policy.
Q9. Which doctor or hospital can I go to?

Q10. How do I file a claim?
Anytime you visit a health care provider, even if you do not pay, we strongly suggest you fill out a claim form so the insurance company has a record of your visit to the health care provider & will be able to pay your claim in a timely manner.
